Introduction to Root Cause Analysis (RCA) methods.
Group exercise: Applying RCA to a sample case.
Day 2 – Corrective Actions and Safety Improvement
Corrective vs. preventive actions in HSE.
Developing effective action plans.
Writing clear and actionable investigation reports.
Communicating findings and lessons learned.
Linking investigations to compliance and continuous improvement.
Promoting a proactive safety culture.
Final exercise: Developing an investigation action plan.
